Digital Gift Card

This is a digital gift card. After the purchase, you will receive a receipt with a link to download a gift certificate. Gift cards can be used as payment for any products and services we have in-office only and expire one year from date of purchase. If the gift card is for another person, please include the name of the recipient of the gift card in the notes at checkout. The gift card will be linked to their account and does not need to be presented. Printed gift cards will not be accepted if the gift card is not tied directly to a patient account.
